The skin beneath the eyes is the thinnest on the face — approximately 0.5 mm compared to 2 mm elsewhere — making it particularly susceptible to visible aging. Dark circles, crepey texture, fine lines, and hollowing are among the most common patient concerns, yet the delicacy of the tissue limits the use of many standard treatments. Hyaluronic acid fillers carry risks of Tyndall effect and vascular occlusion in this area, while aggressive energy-based devices risk hyperpigmentation in the thin periorbital skin.

Why PDRN for the Under-Eye Area

PDRN skin boosters are uniquely suited to periorbital rejuvenation because they improve skin quality from within rather than adding volume or ablating the surface. The mechanism addresses multiple contributing factors simultaneously:

  • Dermal thickening — PDRN-stimulated collagen synthesis increases dermal thickness, reducing the translucency that makes underlying vasculature and pigment visible as dark circles.

Treatment Protocol

  1. Product selection — Low-viscosity PDRN formulations (e.g., Rejuran Healer at 2 mg/mL) are preferred over higher-viscosity or high-MW polynucleotide products to minimize the risk of visible papules in the thin periorbital skin.
  2. Technique — Serial micropuncture or nappage technique using a 32G or 33G needle, delivering micro-aliquots (0.01-0.02 mL per injection point) at 2-3 mm spacing across the infraorbital area.
  3. Depth — Superficial intradermal injection in the papillary to upper reticular dermis. Excessive depth risks intravascular injection or visible product deposits.
  4. Sessions — 3-4 sessions at 2-week intervals for initial treatment, followed by maintenance every 3-4 months.

Expected Results

Patients typically notice initial improvement in skin hydration and texture after the first session, with progressive improvement in dark circles and fine lines over the treatment course. Optimal results are usually evident 4-6 weeks after completing the initial series. Results are cumulative and maintenance-dependent.

Ideal Candidates

The best candidates for periorbital PDRN treatment are patients with mild to moderate under-eye concerns primarily related to skin quality rather than volume loss. Patients with significant tear trough hollowing may benefit from combining PDRN with conservative hyaluronic acid filler placement, with the PDRN addressing skin quality and the filler addressing structural volume deficit.
