Nucleofill Medium is a polynucleotide-based injectable gel developed by Promoitalia, one of Italy's leading aesthetic medicine companies. Unlike lower-concentration PDRN products, Nucleofill Medium delivers a high payload of 15 mg/mL highly purified polynucleotides in a viscous gel matrix, enabling sustained release and prolonged tissue contact time. The polynucleotide chains in Nucleofill Medium are extracted from the gonads of Oncorhynchus mykiss (rainbow trout) and have a higher molecular weight distribution (typically >1500 kDa) compared to salmon-derived PDRN products. This higher molecular weight confers greater viscoelastic properties, allowing the product to function as both a biostimulator and a mild volumizer in the dermal and subdermal planes. Polynucleotides activate the adenosine A2A receptor pathway similarly to PDRN, promoting fibroblast proliferation, extracellular matrix remodeling, and neoangiogenesis. Additionally, the high-molecular-weight chains exhibit potent free radical scavenging activity, protecting tissues from oxidative stress. The gel formulation provides a scaffolding effect that supports new collagen deposition around the injected material as it gradually degrades. Nucleofill Medium is indicated for biorestructuring of moderately aged or photodamaged skin, particularly in patients who require both tissue quality improvement and mild volume restoration.
